CONSENT AND MEDIA RELEASE FORM
PARENT OR LEGAL GUARDIAN ON BEHALF OF A MINOR CHILD
PARENTS AND/OR LEGAL GUARDIANS please carefully read this Consent and Media Release Form (“Consent and Release Form”) and the Official Contest Rules in their entirety. This Consent and Release Form, together with the Official Contest Rules, form a binding agreement relating to your minor child’s participation in the Contest, and/or your agreement, on behalf of your minor child, to be depicted in a work of visual art submitted in the Contest. Your minor child is not eligible to participate in the Contest until a completed and signed Consent and Release Form is received by Divine Women, LLC. Contest Submissions not accompanied by the completed and signed Consent and Release Form will not be considered.
I understand that by participating in the Divine Women Showcase, participants agree to the official rules, and that Divine Women, its agents and employees will have no liability whatsoever for any injuries, losses, or damages of any kind resulting from their use of, or participation in, the performances. Prize is nontransferable and cannot be exchanged for cash. There can be no substitutions for a prize of equal or greater value. Copyright for the entries will remain solely with the artists, though by entering this call for art you are giving Divine Women, LLC permission to use your minor child's entry and showcase performance online. Each entrant (including all members of each entrant that is a group) grants permission to Divine Women, LLC and those acting under its authority to use his/her name, photograph, voice and/or likeness, for advertising and/or publicity purposes, as well as all submissions and performances in any and all media now known or hereinafter invented without territorial or time limitations and without compensation.
Restrictions may apply. By participating in the Divine Women Showcase, a participant agrees to be bound by these official rules, and by all decisions of Divine Women, LLC representatives.
I hereby warrant that I am a legally competent adult and have the full right and authority to execute this Consent and Release Form. If I am a parent or legal guardian of a minor child Participant in the Contest or Depicted Person, I further warrant that I am a legal parent or legally appointed guardian of such a minor child, and that I have the full authority to give consent, to release, and to contract for my minor child.
I also agree to provide any additional information or to execute any additional form(s) that are
necessary for me and/or my minor child to be eligible to participate in the Contest.
